What to expect as you prepare for your visit


Please arrive early. Plan on arriving 15 to 30 minutes before your appointment to create or update your medical records and contact information.

What to Bring

Our Patient Forms

If this is your first visit OR you have not been seen in the past 90 days, you can save time before your appointment by printing and completing the following Patient Forms in the comfort of your own home. These forms are in Adobe Acrobat .pdf format. You will need to download and install Acobe Acrobat Reader to view and print them. Arriving late for appointments

A patient is considered late when he or she arrives after their scheduled appointment time. If you are more than 10 minutes late for your appointment, you may be asked to reschedule your appointment depending on how the doctor’s schedule is running or we will attempt to “work you in” to the doctor’s schedule, understanding that other patients who have arrived on time for their appointments will be given priority to maintain their scheduled appointment time.

Imaging and Specialty Care

Your Medical Provider will decide if you need any Imaging Studies (CT/MRI/X-Rays or other diagnostic test) or if you need to seen by another specialty physician for any of your health concerns. If this is necessary, you will be provided with the information at the time of your visit.

Imaging Studies ordered by our physician will be processed by one of our in-house Diagnostic Coordinators. Depending on your insurance plan, you may be required to contact your Primary Care Physician’s office for scheduling the test. Otherwise, you will be contacted by the facility’s scheduling department directly or by one of our Diagnostic Coordinators.

Lab Work

There are many times that your Provider may request you to have lab work done. Orders are sent electronically to the Laboratory. Please check with the Lab regarding the necessity of an appointment.

After you get the lab work done, the physician will discuss the results at your next appointment. If you were not able to have the lab work done before your return appointment to our office, please let us know so that we can reschedule your appointment as needed.


Your Medical provider may order medications that you need to follow your plan of treatment. We will submit Electronic Prescriptions directly to the pharmacy you provided the information to us for. Please make sure we have the correct pharmacy information on file.


If surgery is indicated, you will be asked to complete the paperwork required to proceed with scheduling. The Surgical Coordinator at the office location you are seen will assist you in completing the following forms:

Our Surgical Coordinator will contact you to schedule the actual surgery date, time, facility and pre-op testing/Medical Clearance as needed.

Any financial responsibility such as co-pay, co-insurance and/or deductible related to the surgery, will be collected pre-operatively. Surgery authorization requests to the insurance company will be handled by our Surgical Coordinator.
